At Together Tied Homes, our care coordination service keeps residents’ needs, appointments, and communication organized — without providing medical or personal care. We offer structure, communication, and follow-up that support independence and connection.

What Care Coordination Includes

1. Communication With Families & Support Systems

   We serve as a central point of contact to keep families updated on:

   • Resident routine changes
   • General wellness observations
   • Important notices or concerns
   • Appointment reminders
   • Community resource updates

2. Appointment & Service Coordination

   We help residents stay organized by coordinating with outside providers:

   • Scheduling or confirming appointments
   • Securing transportation
   • Reminding residents of upcoming visits
   • Communicating special instructions from providers (non-medical)

3. Collaboration With Case Managers & Social Workers

   We work closely with professionals involved in our residents’ care:

   • Sharing general updates on resident participation
   • Supporting housing stability plans
   • Maintaining contact during transitions
   • Assisting with required documentation for agencies

4. Support With Community Services

   We help residents stay connected to community resources such as:

   • Senior centers
   • Benefits programs
   • Food assistance
   • Wellness programs
   • Transportation services

5. Monitoring General Well-Being (Non-Medical)

   While we do not perform medical assessments or treatment, staff are trained to:

   • Observe changes in mood, behavior, or routine
   • Encourage healthy habits and daily structure
   • Report concerns promptly to family or providers
   • Ensure residents are safe, supervised, and supported
